Preventing most (but not all) accidents can be done with 2 things.
The first is fix the ride when needed. There is no excuse for any preventable accident to happen because a park wanted to save a few bucks.
The second is hard to do and that is people need to follow rules. You would figure people would be smart enough to not stand before the first drop, but alas. Same goes for jumping fences for any reason. I mean, i get holding your hands in the air during the ride (even though that is against the rules on nearly every coaster) but people just do not think.
There was an accident a couple years ago on a Togo Standup coaster in Japan where the train derailed and almost flipped over, one person was killed. It was so bad the park was very unsuccesful when it re-opened and now is closed for good.
An ironic accident was after the woman flew off of Perilous Plunge @ Knotts they replaced the restraints, but on the only other one of the same model (Hydro @ Oakwood) they kept the t-bars and continued operating it. A few years later, someone flew out of Hydro and Oakwood ended up changing the restraints. I remember looking at TPR during a Europe trip years ago and all the captions ripped on Knotts for stapeling and having shoulder restraints, not so much .
All 3 Superman Ride of Steels have had accidents, SFNE's brakes have failed and the trains collided before and im unsure which one it was but someone flew out (which resulted in the restraints being modified to include shackles).
At Disneyland on Big Thunder Mountain the front car derailed and fell off resulting in a death.
At SFWOA an operater fell through Batman Knight Flights station when the floor went down, dont remember if he died or not.
At SFMM a few years back an operator was walking through the ride area during the block check and was killed by a moving train.
Last year a Chance Yo-Yo ride in California (cheap versions of Wave Swingers like Whirligig) collapsed while it was running.
Several people have been hit by moving trains while trying to retrieve an item they lost on a ride: most recent ones I can think of were Ninja at SFMM and BTR at SFOG (that one died).
In 2007 Son of Beast at Kings Island de-railed. I dont remember if there were any deaths but they got new trains (bought trains from a different park) and removed the loop because the new trains werent designed for the loop.
Darien Lake SROS lost a rider like weeks after it opened which resulted in the addition of seatbelts.
SFNE SROS lost a rider (rather gruesomely, the body was not recovered in one piece) which resulted in the shackles, and now new trains for SFNE. Also SFNE's Superman had it's liftchain break several times opening year, though it was determined that the chain was defective and they replaced the entire chain rather than just the links that break.
There are way to many fatal accidents to list in the past 10 or so years.

Also for SOB, the train never derailed, a bent broke resulting in a pothole to end all potholes on wood coasters. The train did make it back to the station normally with everyone complaining of aches, pains and bruises.

Probably nothing serious but I think with the age of the rides now there will be another accident (or more) -Another flat fair ride and some hick ass fair will break apart or collapse (a annual type of accident) -Some one will fall off of a ride (most likely in America). People are getting dumber = people dying. I think something will happen on a wooden coaster this year. -pre condition, not reading 5-10 signs before getting on a ride, death with the usual family trying to sue the park.

Witnesses told investigators they saw Fellner standing up in the car as it neared the drop. An investigation after the accident showed that Fellner's seat restraints were working correctly and no mechanical deficiencies were found on the coaster. The coroner's office ruled Fellner's death as accidental. However in 2005, Fellner's family members filed lawsuits against both Holiday World and the Philadelphia Toboggan Company; the manufacturer of the coaster trains. The lawsuits were settled out of court in 2007. Terms of the settlements were undisclosed."
Basically, this is why they no longer do Stark Raven Mad events and moved to HoliWood Nights.
No one has mentioned the accident on Mind Bender... By far one of the worst!
"On the evening of June 14, 1986, after the yellow train (train #1) completed the second inverted loop, it encountered one of three areas of uplift before the third and final loop. Missing bolts on the left inside wheel assembly of the last car of the four car train caused the bogey assembly to disengage the track with a full load of riders. This caused the final car to fishtail wildly, disengaging the lap bars as it collided with support structures, thereby throwing off passengers and losing speed. The train entered the third and final inverted loop, but did not have the speed to complete the loop. The train stalled at the top, then slid backwards, crashing into a concrete pillar. Three people were killed during the accident and a fourth man was severely injured."
if it's the pic I'm thinking of and have, it's the train all smashed up with a bleeding corpse covered with a sheet on the ground under what's left of the smashed up car.

You are pretty accuarte. Someone had a heart condition and went on Firehawk, problems with SOB even though it wasn't the rides fault, and probably a ride at a fair someone got injured most likely.
