Train Accidents - Possible Solution

It seems so simple, but you have people passing out driving trains, and wouldn't it make more sense to hire more staff to operate the trains. Planes have 3 pilots, and than hostesses, but the L only has 1 person driving. For the Monorail pilot at Disney, maybe that accident could have not happened if their was another Monorail guy that said hey, your not supposed to go, or you are on the wrong track. When dealing with people's lifes, you shouldn't be cutting back.

It wasn't really the pilots fault in the monorail accident. Two CM's who monitor all trains at shop decided to leave their post and put a janitor in charge who had no idea what he was doing. With a system like MAPO, you don't need two pilots on one train.

I'm not familiar with other types or rail transportation though, so I have no comment.

I know all freight trains have a button that needs to be pressed every 30-60 seconds, if it is not, it emits a warning tone and you have another 30 seconds to push it. If it is still not pushed, it engages the emergency breaks. This is to prevent run away trains if the engineer becomes incapacitated or asleep.
